Rescheduling/Cancelling Your Exam
To provide each and every patient with a thorough, unhurried physical exam and extensive time with the physicians, we limit the number of patients seen each day. To maintain our high standard, the following Cancellation Policy is in place.
Seven days’ advance notice is required for all cancellations and/or rescheduling of appointments to avoid incurring a fee. If you must reschedule or cancel your appointment, please call 866.906.2667 (972.560.2667).
Less than seven days’ advance notice
$250 non-refundable cancellation fee
Less than 24 hours’ advance notice or a missed appointment
$500 non-refundable cancellation fee
More than two cancellations with less than seven days’ notice during a calendar year
$500 non-refundable cancellation fee
