Cancellations
If you must cancel your appointment, please call at least one week in advance. Appointments may not be canceled by e-mail.
Payment/Insurance
Full payment is appreciated the day of service. The business office will assist you in filing an insurance claim or give you the necessary information to file a claim. Because Cooper Clinic procedures are preventive in nature, the costs of the procedures may not be covered by insurance or Medicare. We accept Mastercard, Visa, Discover, American Express, a personal check, debit card, or cash.
