Cancellations
If you must cancel your appointment, please call at least one week in advance. Appointments may not be canceled by e-mail.
Payment/Insurance
Because Cooper Clinic procedures are preventive in nature and typically out-of-network, the costs of the procedures may not be covered by insurance or Medicare. The business office will assist you in filing an insurance claim or give you the necessary information to file a claim. Full payment is appreciated the day of service. We accept Mastercard, Visa, Discover, American Express, debit card, personal check or cash.
