Diabetes/Pre-Diabetes Management and Prevention

Cooper Clinic's individualized, Nutrition for Life approach to diabetes management and prevention uses the Association of Diabetes Care and Education Specialist (ADCES)7 Self-Care Behaviors to help you manage your diabetes or pre-diabetes or prevent developing diabetes. Diabetes Care and Education Specialists have developed seven key areas of care to help you set priorities and work with you to improve your own management:

  • Health coping
  • Healthy eating
  • Being active
  • Taking medication
  • Monitoring
  • Problem solving
  • Reducing risks

Whether you are recently diagnosed with diabetes or pre-diabetes, have been living with diabetes for some time or you want to prevent developing diabetes/pre-diabetes, Cooper Clinic Nutrition has experienced Certified Diabetes Care and Education Specialists (CDCES) to help you. Our CDCES Registered Dietitian Nutritionists can provide you with practical tools to meet your treatment goals.

Diabetes/Pre-Diabetes/Diabetes Prevention Package

A six-session program based on the ADCES7 Self-Care Behaviors includes:

  • A one-hour initial consultation with an experienced Registered Dietitian Nutritionist/Certified Diabetes Care and Education Specialist (CDCES)
     
  • Personalized goals with an individual eating plan, exercise guidelines and self-monitoring of blood glucose
     
  • Five follow-up sessions, 30 minutes each

Research shows additional follow-up helps with more successful healthy habit formation and behavior change.

Individualized Diabetes/Pre-Diabetes/Diabetes Prevention Nutrition Consultation

  • A one-hour initial consultation with an experienced Registered Dietitian Nutritionist/Certified Diabetes Care and Education Specialist (CDCES)
     
  • Personalized goals with an individualized eating plan, exercise guidelines and self-monitoring of blood glucose
